DATA PROTECTION POLICY
Information obtained by Haven Therapies will be collected lawfully and in accordance with the General Data Protection Regulations (GDPR)
With changes implemented in May 2018 under the General Data Protection Regulations (GDPR) this gives individuals the right to be informed about the collection and use of their personal data, which includes the collection of health information. This will be covered in further detail during your first consultation visit at Haven Therapies

Consent
By filling in the enquiry form on this website, you are giving your consent to Haven Therapies to hold your personal information and to make contact. By signing the initial consultation form during your first visit, you are giving your consent for Haven Therapies to hold your personal information. The information which you supply on the forms will be held together with any clinical records or purchasing information, as applicable. We will use your information to give you the best possible service and for insurance purposes.

Storing your information
You information is stored in different ways by our practitioners. Digitally or hand written forms. The hand written forms are kept in a locked cabinet, accessed only by your practitioner, whereas digital information is kept on a secure site

Retaining your information
You have the right to ask us to anonymise your records at any time if you wish to terminate our working relationship. We are required by law to retain adult records for eight years following our last date of contact and will destroy them with a shredder after that date. Children’s records are retained until their 25th or 26th birthday, depending on the exact age of the child when their treatment ended.

Accessing your information
Your records are accessed by your practitioner for clinical purposes. You have the right to request a copy of your records at any time. These will be sent by email, in a Microsoft Word document attachment.

Sharing your information
Haven Therapies’ practitioners will never share your information without your permission – it will be kept strictly confidential unless there are legal grounds for me to disclose something which you have told us to a third party. We shall always inform you if we feel that disclosure is necessary and seek your permission and co-operation first.
With your agreed and written consent there may be instances where your medical information may be shared by the various healthcare providers involved in your on-going care and treatment.

Complaints
If, for any reason, you are unhappy with how any of our practitioners are handling your data, please raise your concerns with the clinic first, so that we can seek a resolution. If following this you are still not satisfied, then you have the right to complain to the Information Commissioner’s Office (ICO).
We may amend this privacy policy from time to time when required by legislation to do so. You should check back periodically to check for any changes. This privacy policy is effective from the introduction of GDPR on 25th May 2018.
